Our Platinum Care Membership is tailored to give you freedom AND control over your healthcare

Membership Includes:

  • Annual Check-ups
  • Counseling and Guidance (Nutrition, Development, Sleep)
  • Developmental and Behavioral Screenings
  • Evaluation and Medical Management of Chronic Conditions
  • Expedited Access
  • Management of Minor Lacerations (stitches, staples, glue)
  • Remote Visits / Virtual Care
  • School / Camp / Sports Physical Forms
  • Unlimited Sick Visits

Plan Details:

  • All Plans have a $100.00 Registration Fee
  • One registration fee per family
  • Membership is recurring until canceled
  • 3 Month minimum commitment is required

Membership Pricing:

  • INFANTS
  • $150 per month for 0-12 months of age

  • TODDLERS
  • $125 per month for 1-2 years of age

  • PRE-K
  • $100 per month for 3-5 years of age

  • KIDS
  • $75 per month for 6-21 years of age

  • ADULTS
  • $100 per month for 22+ years of age

  • FAMILY
  • $400 per month for family of 4
  • $75 per month for each additional member
